We strive to maintain our records as accurately as possible.  Changes that we need to know about include:

• Legal name changes
• Address
• Home Phone
• Cell Phone
• New employer
• Employer phone
• Email address
• Emergency contact person and contact number
• Health issues/medications
• Health insurance provider
• Physician name and phone 

You can make the changes in several ways.

• Log into Skyward Family Access from our website.  Click on For Parents/Students.  Click on Skyward Student/Family Access Link and make the changes yourself.
• Send a note to school with your child.
• Email the database secretary at jrw@saukpr.k12.wi.us.
